Choose one of the following types of forms, either Microsoft Word or Adobe PDF. If you want to keep a copy of the form on your computer, you should choose Microsoft Word unless you have a full version of Adobe Acrobat.

	Microsoft Word (fillable)	Save this file to your computer by right clicking on the link, then selecting "save link as" or "save target as". You can then type in information while the form is displayed on your computer monitor.   Save the document and either fax it back or attach it to an email.
	Adobe PDF (fillable)	Save this file to your computer by right clicking on the link, then selecting "save link as" or "save target as". You can then type in information while the form is displayed on your computer monitor. However, you cannot save data typed into this form. You must print a hard copy of the from and fax it to us.
